Alcohol after weight-loss surgery

This is the one worth reading twice. Alcohol behaves differently after these operations, and the risk of a drinking problem rises measurably in the years afterwards. The research below is large, prospective, and long.

It reaches a higher peak, faster

The amount you could once manage is not a guide. Absorption changes after a sleeve and after a bypass, and people are commonly surprised by how quickly they feel it.

What happens in the second year

In a prospective study of nearly 2,000 people across ten US hospitals, "the prevalence of AUD symptoms did not significantly differ from 1 year before to 1 year after bariatric surgery (7.6% vs 7.3%), but was significantly higher in the second postoperative year (9.6%)" (King WC et al., JAMA, 2012 · doi:10.1001/jama.2012.6147).

The first year is not the test.

Over five years, after a bypass

Following the same group further: the five-year cumulative incidence of new alcohol use disorder symptoms was 20.8% after Roux-en-Y gastric bypass and 11.3% after a band. "Undergoing RYGB versus LAGB was associated with twice the risk of incident AUD symptoms. One-fifth of participants reported incident AUD symptoms within 5 years post-RYGB" (King WC et al., Surgery for Obesity and Related Diseases, 2017 · doi:10.1016/j.soard.2017.03.021).

Over twenty-five years

A Swedish study followed 2,007 surgical patients and 2,040 matched controls for a median of 25 years. Compared with controls, the adjusted risk of alcohol use disorder was 5.07 times higher after gastric bypass, 2.34 after banding and 2.28 after vertical banded gastroplasty (Sjöholm K et al., British Journal of Surgery, 2025 · doi:10.1093/bjs/znaf211). Alcohol-related mortality after gastric bypass had a sub-hazard ratio of 6.18 (Sjöholm K et al., British Journal of Surgery, 2025 · doi:10.1093/bjs/znaf211).

What people usually do about it

Most teams ask for none in the first year. After that it is a conversation with yours, and knowing the numbers above is most of what makes that conversation a real one.

If drinking has become something you think about, that is worth raising with your team, or with a national helpline. It is a common thing to happen and an early one to fix.
