Pregnancy after bariatric surgery: timing and monitoring
Pregnancy after one of these operations is planned rather than happened upon. Two things make the timing matter: weight is changing quickly, and nutrition is still settling.
The published interval
The guideline says candidates "should avoid pregnancy preoperatively and for 12 to 18 months postoperatively" (Mechanick JI et al., Obesity, 2013 · doi:10.1002/oby.20461).
The reasoning is straightforward. The fastest weight loss is in that window, nutritional gaps that were not obvious before surgery become obvious in it, and both are worth resolving first.
Fertility can return sooner than expected
For a lot of people, cycles regularise and fertility improves after these operations, sometimes quickly. That makes contraception a conversation to have early rather than later, and one worth having even if it had not been necessary before.
When you do plan one
Nutritional monitoring through the pregnancy is part of the picture. Your surgical team and your maternity care plan it between them, and the supplement schedule usually changes. Folate in particular is different for women of childbearing age, at 800–1,000 mcg a day rather than 400–800 (Parrott J et al., Surgery for Obesity and Related Diseases, 2017 · doi:10.1016/j.soard.2016.12.018).
Ask both teams to talk to each other. It is a reasonable thing to ask for and it makes the rest easier.
